Drop History altogether - not only does it not interest you, but it's also making you stressed.
Stick to the subjects that you like as you'll have a higher chance of doing well in them.
Try to allocate some of your study time towards maths. If you study for about 2-3 hours a day for 2 weeks then that should be enough (considering that you're doing 5.2)
I suggest you post questions that you struggle with on this website while you're revising. Plenty of people here can help.
As mentioned by @Sunaina123 studying notes for a maths exam doesn't really help. You should practise as many questions/exercises as possible before your exam.
A friend of mine got just above 92 ATAR and he also did Advanced Maths, Standard English and Business Studies. If you think SDD will be too hard for you I think doing IPT instead might be a better option. It doesn't scale great but it's easy to get a band 6 in it as long as you study regularly...
Doing well in 4U is very helpful if you want to get 99+. But even if you don't do it, you can still get this ATAR by acing your other subjects.
Give it a try and study hard. If it ends up being too much then you should probably drop it.
